I understand that this is an elective cosmetic treatment. The expected benefits, treatment area, number of sessions that may be needed, and aftercare instructions will be explained to me. Results vary and are not guaranteed.

I understand that possible effects and complications may include discomfort, heat or burning, redness, swelling, tenderness, spotting or bleeding, discharge, urinary symptoms, infection, burns, blistering, scarring, changes in skin pigmentation, altered sensation, pain during intercourse, persistent pain, tissue injury, no improvement, worsening symptoms, or the need for additional medical care.

I understand that alternatives may include no treatment, evaluation by a gynecologic clinician, pelvic floor physical therapy, or other medical or surgical options appropriate to my concern. This cosmetic treatment does not replace routine gynecologic care or evaluation of abnormal symptoms.

I confirm that I have disclosed my relevant medical history, medications, allergies, pregnancy status, symptoms, and prior procedures. I agree to follow all pre-treatment and aftercare instructions and contact Lavender Med Spa promptly if I develop severe or worsening pain, heavy bleeding, fever, foul-smelling discharge, difficulty urinating, signs of infection, or another concerning symptom.

I have had the opportunity to ask questions. I understand that I may decline or stop treatment before it begins. I voluntarily authorize the treating professional at Lavender Med Spa to perform this treatment.
